Disease Management

Care Paths are the disease management programs specifically designed for the treatment of the most common home health diseases and diagnoses. Our clinicians (nurses, physical, occupational and speech therapists) initiate the Care Paths (protocols) upon admission and personalize the care to the patient’s needs. The Care Path consists of assessment, treatment and goals expected for the patient while on services. In addition, there are easy to follow teaching guides for the patient and/or family to follow. Some of our most commonly used Care Paths include:
  • Congestive Heart Failure
  • Diabetes
  • Cerebrovascular Accident (Stroke)
  • Chronic Obstructive Pulmonary Disease
  • Pneumonia
  • Cardiac Recovery
  • Wound
  • Cancer

Many more less common diseases and diagnoses
Note: Any individuals who have frequent problems with long standing diseases or medical problems may benefit from our Care Path disease management programs. Please do not hesitate to contact us with any questions you may have.
